The Philippine Health Insurance Corp.
(PhilHealth) had amended the RA 7875, which is now known as the National
Health Insurance Act of 2013.
The amendments include that every Filipino
is required to be a member of PhilHealth. Additionally, subsidy from the
government will be given to those who are identified by the Department of Social Welfare and Developmnt as indigenous
groups.
PhilHealth
membership among employees is mandatory, including household-helpers. For newly
hired employees, follow the succeeding instructions in order to acquire your
PhilHealth Numbers (PIN).
1. If you are near your local
PhilHealth office, you can visit them and ask for a PhilHealth Member
Registration Form (PMRF) or you can download one here.
2. Acquire a copy of your birth
certificate (insert how to get NSO link) or your baptismal certificate.
3. For those who have dependents, it
is advisable that you bring and submit NSO birth certificate of your dependents.
4. In most of the companies, you may
submit the requirements you have filed to the HRM Department as they will be
the one to submit to PhilHealth. The Report of Employee-Members (Er2 Form) will
also be submitted by your employer.
5. When the process is done, your
employer will then give you your PhilHealth Identification Card (PIC) and your
copy of the Member Data Record (MDR).
